Form profiles: your logo and contact details
Create your brand identity once, attach it to any form — update it centrally.

A form profile is a reusable identity block shown at the top of your public forms: logo, name, description, e-mail, phone, address and up to 10 links.
Steps
- Go to Settings → Form profiles and create a profile.
- Upload your logo (up to 10 MB — it's resized automatically), fill in the contact details you want respondents to see.
- In any form's Settings → General, select the profile.
Respondents see the profile as a professional header — they know immediately who is asking, and how to reach you with questions. That transparency measurably improves completion.
Change the profile once — every form using it updates. Perfect for a rebrand, a new phone number, or seasonal contact hours.
Plan note: Free includes 2 profiles; Pro and Business have no limit — useful if you run forms for multiple brands, locations or clients.
Tip
Keep the description to one sentence about why you're asking ("We use this form for new patient registrations") — it reads as care, not marketing.
