Paste it into your booking confirmation, put it on your website or show the QR code at reception. The form carries your studio's name and logo.

You open the vault
Responses decrypt in your browser with the Vault key. Tag them by treatment or by therapist.
Read before, file after
Read the answers before the client arrives. Export one response as PDF for the client file, or a set as Excel or ZIP — built in your browser.
Confidentiality, handled
Before a massage, a tattoo or a first training session, your clients tell you about medication, pregnancy and past operations. With Schweizerform, their answers are encrypted in their browser and only your studio can open them. We also offer a data processing agreement.
Health answers only your studio can open. Encrypted on the client's phone, decrypted only with your Vault key. We store ciphertext — uploaded files included, under random names.
Questions only where they apply. Conditional questions ask about an operation only when the client says they had one; answers to hidden questions are never stored.
File it or delete it. Export a response as a PDF for the client file, or delete it — its encrypted files are deleted with it. On every plan.
